J C On-Site Testing LLC created this website to offer our services to our potential clients so that we can ensure safety.  We deal with many people’s personal data and we take pride that we keep that classified.  We do not collect your data to sell to anyone; the information that we collect, you voluntarily provide to us, on our contact us page which is your name, city, phone number, and email address so that we can get a hold of you to see what we can provide for you.  No mobile phone information will be shared with third parties/affiliates for marketing/promotional purposes.  All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.  We do not record your IP address, we do not have tracking or identifying cookies, we do not conduct any analysis of who visits our site, we do not allow third-party tracking of any kind on this site.  This website contains links to other sites.  Please be aware that we are not responsible for the content or privacy practices of such other sites.  We encourage our users to be aware when they leave our site and to read the privacy statements of any other site that collects personally identifiable information. Our clients are always in charge of their information; if you’re not comfortable inputting your information on our contact page, feel free to give us a call.

J C On-Site Testing LLC (“Company,” “we,” “us,” or “our”) is committed to protecting the privacy and confidentiality of our clients. This Privacy Policy explains how we collect, use, and safeguard personal information.

- Personal information provided by clients (e.g., name, birth date, phone number, email, address). 

- Employer or attorney contact information when services are scheduled on behalf of another party. 

- Specimen collection data (urine, hair, oral fluid, breath, DNA) as required for collection. 

- Payment information when services are purchased. 

 

2. How We Use Information

We use the information we collect to:

- Schedule and fulfill drug, alcohol, and DNA collection services. 

- Provide results to the authorized employer, attorney, or individual client. 

- Maintain compliance with federal and state regulations. 

- Process payments and issue receipts. 

 

3. Confidentiality of Test Results

- Test results are released only to the designated employer representative (e.g., employer, attorney, or client) once we receive them from the Medical Review Officer. 

- We do not share results with unauthorized parties. 

- All records are maintained securely in accordance with HIPAA and DOT confidentiality standards. 

 

4. Data Security

We take appropriate measures to safeguard personal and testing information against loss, misuse, unauthorized access, disclosure, alteration, or destruction. 

 

5. Sharing of Information

We do not sell, rent, or trade personal information to third parties. Information may be shared only when:

- Required by law, subpoena, or government regulation. 

- Authorized in writing by the client. 

- Necessary to comply with DOT or other regulatory standards. 

 

6. HIPAA & DOT Confidentiality Notice

J C On-Site Testing LLC is a collection service, not a medical provider, healthcare facility, or laboratory. We do not diagnose, treat, provide medical care or test any specimens. All testing information is handled in compliance with: 

- The Health Insurance Portability and Accountability Act (HIPAA) confidentiality requirements. 

- The U.S. Department of Transportation (DOT) regulations governing drug and alcohol testing (49 CFR Part 40). 

These regulations ensure that test results and client records remain strictly confidential and are disclosed only to authorized parties. 

 

7. Client Rights

Clients may request copies of their personal data or test results, subject to identity verification. Clients may also request corrections to inaccurate information. 

 

8. Consent

By booking our services, you consent to the collection and use of information as described in this Privacy Policy.

Refund & Cancellation Policy

At J C On-Site Testing LLC, we provide professional, mobile drug and alcohol collection services. Because our services require staff scheduling and preparation in advance, we have the following refund and cancellation policy. We understand that schedules can change, therefore the following policies ensure clarity for both our clients and our team:
Cancellations & Rescheduling

  • You may cancel or reschedule a service at no charge with at least 24 hours’ notice before your scheduled appointment.

  • Cancellations made less than 24 hours in advance will result in a fee up to 50% of the scheduled service price.

  • If our collector arrives on-site and the client cancels or is unavailable, the full-service fee will be charged.

  • No-shows will be charged in full for scheduled service.

Refunds

  • If for any reason we cannot provide services ordered, please request a refund in writing to cari@jc-testing.com

  • Refunds are issued to the original payment method within 5-7 business days of an approved cancellation.

  • If J C On-Site Testing LLC cancels your appointment, you will be offered the choice of a full refund or a rescheduled appointment at no additional charge.

Chargebacks

  • I agree that no “charge-backs” will be issued prior to making contact with J C On-Site Testing LLC to settle any disputes and J C On-Site Testing LLC requires written agreement to perform a “charge-back”.

Payment Processing Fee

  • All credit card transactions are subject to a 2.9% processing fee, which reflects the actual cost of card acceptance in accordance with federal and Wisconsin law. This fee is non-refundable.

Service Delivery Policy

At J C On-Site Testing LLC, we provide mobile drug, alcohol, and DNA collection services.  Our collectors travel to the client’s designated location or agreed collection site at the schedule appointment time, we are committed to delivering our services promptly and professionally.

Booking Confirmation

  • All appointments and locations are confirmed within 1 business day of booking.

On-Site Service

  • Services are delivered on-site at the address provided by the client during booking.

  • Clients must ensure a safe, suitable environment for collections if they are not utilizing J C On-Site Testing’s mobile vehicle.

  • Mobile drug and alcohol collection typically takes 30 minutes per individual.

  • Instant collection results are provided immediately after collection.

  • Laboratory-based results are usually available within 3 business days, depending on the type of test and laboratory processing times.

Compliance Standards

  • All services are performed in accordance with Department of Transportation (DOT) regulations and other applicable workplace compliance standards.

  • Because we are a certified professional collector, no physical products are shipped.

  • All lab-based specimens that we collect are shipped via FedEx Next Day to a certified DOT-approved laboratory for analysis and result reporting.

Delivery of Results

  • Test results and reports are delivered electronically once J C On-Site Testing LLC receives them from the Medical Review Officer.

  • For employer compliance programs, results are reported directly to the designated employer representative in compliance with DOT and federal confidentiality standards.

Terms & Conditions

1. Services

We provide third-party drug, alcohol, and DNA collection services. We do not diagnose, treat, or provide medical care of any kind. We operate strictly as a collection service for employers, individuals, and legal representatives.

 

2. Payments

Payment is due at the time of service unless prior arrangements are made. We accept cash, check, ACH, and major credit/debit cards. A 2.9% processing fee applies to all card transactions. Your payment information will be handled securely and will not be shred with others. For your safety, your credit card information is not stored on this web site.

 

3. Refunds & Cancellations

Please see our Refund & Cancellation Policy for details.

 

4. Confidentiality

All collection information is handled in compliance with HIPAA, DOT, and applicable state/federal confidentiality standards.

 

5. Compliance Standards

All specimen collections are preformed in compliance with DOT regulations or applicable workplace and legal standards.

 

6. Limitation of Liability

We are not liable for any indirect, incidental, or consequential damages resulting from use of our services, including but not limited to employment decisions or legal outcomes. Clients are responsible for providing accurate information at the time of service.

 

7. Consent

By booking, you consent to the collection and handling of specimens as outlines in our Privacy Policy.

 

8. Governing Law

These Terms are governed by the laws of the State of Wisconsin.
